The Civilian Incident Report is for reporting lost/damaged property. For a detailed explanation of what that means please read the related service page before proceeding.

Options For Completion

The form can be completed online by clicking the “fill out form” tab above. If there is a spanish version of the form online, there will be a tab next to that which says “espanol.”

If you do not want to complete the form online, please come into the station to complete a paper form.

What to Expect After Submitting

Upon submitting this online, you can expect:

  1. You will receive an email confirmation of your submitted form including a copy of your submission. At that point the form is under review.
  2. A Police Department employee will review your form.
  3. Once your form submission is approved, a copy of the report filed upon your behalf will be emailed to you.
  4. If your form submission requires further follow-up, a police officer will contact you for further assistance.
